FREE CONSULTATION VS. NEW PATIENT APPOINTMENT

A free 15 minute consultation is offered to all prospective patients because when looking for a new practitioner, Dr. Levy wants to make sure the right match is made. The free consult allows for a small meet-and-greet with you and Dr. Levy via phone or in person. During this time you can ask questions that can help you determine if Dr. Levy is the right practitioner for you as well as how his treatments could potentially benefit you.  This is also a great opportunity to discuss the process of what new patient and follow up visits may entail and which modalities of care are desired and potentially appropriate for you.

A new patient appointment is intended for patients who wish to establish treatment with Dr. Levy.  All new patient Naturopathic and Acupuncture visits are 90 minutes in length which allows for a comprehensive history, relevant physical exams, and treatments to occur.

WHAT DO I NEED TO BRING TO MY NEW PATIENT APPOINTMENT?

For insurance patients, we require that you bring in a copy of your up-to-date insurance card.

Please bring any medications or supplements that you are currently taking to your visit.  Alternatively, you can take pictures of them on your phone and bring that to the visit but please remember to photograph the ingredients on the supplements so we can see them.

​

We do require that a credit card be placed on file for all patients so please bring that to your visit.  We do not run any charges on your card without your approval.

​

Additionally, please make sure your paperwork is filled out. If you do not know how to access your patient portal or paperwork please contact the front desk.

​

For those seeking treatment for a motor vehicle accident (MVA) or worker’s compensation, please bring the following information:

  • Name of your insurance plan, phone number, and address

  • Claim representative’s name and phone number

  • Claim number

  • Your attorney’s name and phone number (if applicable)

DO YOU SELL SUPPLEMENTS?

Yes, we sell a wide variety of supplements, eastern and western herbs, and homeopathic remedies.  We prefer that you request them through the online portal, however you may also call the receptionist to request them.  Please make your requests at least 24 hours prior to your desired pickup time to allow sufficient time for preparing them. 
 
It is important to note that our receptionist is not qualified to give medical advice. Please save questions regarding dosage, and other specifics for Dr. Levy to answer.

PARKING & ADA ACCESSIBILITY

Yes, there is a large parking lot directly across from our location on the west side of 12th Avenue that is easily identified by a very large blue "P" marking the entryway to it.  This lot is maintained by the city and it is free, well lit, and it has approximately 50 spaces available.

We do have an ADA ramp that leads to the basement floor of the clinic which has treatment rooms.  Dr. Levy's clinic space is located on the 1st floor of the clinic so if you need ADA accessibility just let us know when you sign up for your appointment and Dr. Levy will make arrangements for your visit to occur on the basement floor.

INSURANCE INFORMATION

If you’re interested in seeing Dr. Levy for a particular service, and are curious if he can take your insurance, please call the front desk to inquire. We also very strongly recommend that you check with your insurance carrier to make sure that the services you are interested in are covered in your plan.  While Dr. Levy may be in-network with your insurance company, the services you seek may not be covered in all your carrier's plans, and it is the patient's responsibility to ensure that they have coverage.
